If you’re a healthcare provider, covered entity, or business associate looking for better form workflows, we’d love your feedback while we identify bugs, usability issues, and areas to improve.
Form Vessel includes HIPAA-aligned safeguards, but we will not offer a BAA during the beta. For that reason, please do not use the beta to collect, store, or process PHI or other sensitive patient data. We recommend using test data only until the beta ends.
We built Form Vessel to make creating and managing forms simple for any team, including intake, consent, screenings, referrals, and more. This beta helps us validate real workflows, catch issues early, and prioritize the improvements that matter most to healthcare organizations.

Form Vessel is offering the same core feature set in the beta that we plan to offer in our paid plan at launch. We’re keeping it simple. One all-inclusive plan. No feature tiers to compare while you evaluate whether it fits your workflow:
* HIPAA compliance cannot be achieved by software alone. Form Vessel is designed to support compliant workflows, but your organization is responsible for meeting all HIPAA requirements.
† A Business Associate Agreement (BAA) is required for vendors to handle PHI on your behalf. A BAA will be available after the beta concludes. Until then, please use test data only.
Joining the beta is easy. Enter your email to sign up, then start building your first form. You can create unlimited forms and publish them in two ways: generate an embed code to add a form to your website or share a direct link that opens the form in a browser.
When the beta ends, we’ll reach out to learn what you liked, what could be better, and share next steps for teams that want to continue using Form Vessel.
We hope that once you’ve tried Form Vessel, you’ll want to keep using it. For teams that continue after the beta, we’ll migrate your beta forms and data into the full product so you can pick up right where you left off, without rebuilding anything.
If you decide not to continue, we’ll provide a way to export your data before the beta environment closes.
